Respiratory ACP

We would love to hear from experienced ACPs with respiratory and physical assessment skills to support this service based in Shropshire.
This role is to deliver the virtual ward service within the respiratory community teams, supported by the trust ICS, working in partnership with primary and secondary care for the benefit of the patient, enhancing community care delivery.
Your own car is essential as you will require to travel around Shropshire, Telford and Wrekin as needed.
This is opportunity is to cover weekends and bank holidays 08:00am – 18:00pm with potential week days available.
