Hallam Medical have an exciting opportunity for Community Nurses (Band 7) to support a brand new Admission Avoidance Pilot Scheme in the Shropshire area.
We would love to hear from experienced Community RGNs to work with the Admission Avoidance Team to help prevent admissions in to the acute trust during the winters pressures.
You must be able to complete advanced physical assessments, as you will be responsible for the assessments of patients care and develops, implements and evaluates a programme of care using specialist, analytical and judgemental skills based on sound research findings to assist with admission avoidance. Assessment and interpretation of complex facts and situations is also required when caring for acute and chronic patient conditions.
We are looking for experienced Community Nurses who have the following skills:
– Advanced Physical Assessment Skills qualification
– Non-Medical Prescribing qualification
– Own car is essential
This service operates Monday – Friday 10:00am – 18:00pm with the occasional weekend.
