Community Nurse (Band 7/8a)

We are looking for Band 7/8a Community Nurses to join the Medicines Support Team in Huddersfield.
As part of this role you will be undertaking Long-term-condition reviews for housebound patients on behalf of GP practices. These will include Diabetes, Heart Failure, COPD, CHD, Asthma and hypertension in line with QOF.
You will take observations and sign post as appropriate, as well as escalate any deteriorating patients.
Access to your own car is essential as you will be travelling between patients homes.
