The conference is ran by the South Yorkshire Primary Care Workforce and Training Hub.

The conference aim is for PCN clinical leaders and managers, federations, workforce leads and clinicians interested in the development and delivery of PCN staff and services, collaborating to discuss the development of new ways of working, planning and action.
It will include South Yorkshire approaches to Frailty, Chronic Pain Pathways, Mental Health and advancing Personalised care. There will be opportunity to explore team impacts and models delivering care in specialist pathways and Multidisciplinary teams including Pharmacy, Physio, PA, Paramedic, Dietician, ACP, Occupational Therapy, and Personalised care roles.
