Advanced Clinical Practitioner (ACP)
Posted 25 November 2024 by LyndseyHallam Medical is looking for Advanced Clinical Practitioners to provide personalised, high-quality care to patients in their own homes as part of the Scarborough Virtual Frailty Ward.
In this role you will be working as part of a MDT with clinical support and oversight remotely from a GPwER in Geriatric Medicine based in the Frailty Hub. We are keen to speak with ACP’s to help improve the quality of life for frail patients in their own homes, delivering compassionate and effective healthcare.
There are 4hr, 6hr and 8hr shifts available with flexibility on hours. We are looking for clinicians who would like to book regular shifts over the coming months.
To Apply:
- Qualified ACPs with experience in managing frail and elderly patients.
- Strong clinical skills and the ability to work independently while collaborating with the MDT.
- Passionate clinicians who can commit to regular shifts to build continuity of care.
