Hallam Medical have an exciting opportunity for Advanced Clinical Practitioners (ACP) to support an Urgent Care Centre in Rotherham.

Are you an experienced and forward-thinking Advanced Clinical Practitioner (ACP) looking to work within a dynamic and supportive urgent care environment?
We are seeking motivated ACPs to join our multidisciplinary team at the Rotherham Urgent Care Centre, delivering high-quality, patient-centred care across a range of integrated services.

About the Role

As an Advanced Clinical Practitioner, you will play a vital role in providing clinical assessment, diagnosis, treatment, and management of patients presenting with urgent and primary care needs.
You will work autonomously and collaboratively within a skilled team of clinicians and practitioners across:

• Urgent Care – assessing and managing patients presenting with minor illness and injury.
• Primary Care – delivering same-day and routine appointments.
• Telephone Triage – providing remote clinical advice and directing patients to the most appropriate care.
• Home Visiting Service – offering high-quality care to patients in the community.

You will utilise your advanced clinical skills to make informed decisions, prescribe safely, and ensure continuity of care across the urgent care pathway.
