Community Mental Health Nurse (Band 6 RMN)
Posted 30 December 2025 by TiaHallam Medical has exciting opportunities for Band 6 Community Mental Health Nurses to join a Dementia Assessment and Diagnosis Service across Telford and Shrewsbury.
We are seeking experienced and compassionate NMC Registered Mental Health Nurse with to deliver high-quality clinical services, supporting patients and their families in the community for a 3 month period.
About the role:
- Working in an identified dementia community mental health pathway, with the flexibility of working across other mental health pathways, dependent on skill and service
- Delivering a high quality clinical service to patients and their
- Managing a complex caseload and responsible for clinical and caseload supervision of other team members where
- Responsible for providing the nursing process to a specialist clinical
- Delivering nursing interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related
Requirements:
- Registered Mental Health Nurse with active NMC registration.
- Dementia experience or old age experience to support the team with triage and urgent assessments in the community.
- Driving License and access to a vehicle
- Proven ability to work autonomously within a community setting.
Shifts available: 09:00 – 17:00 for a 12 week period
