Senior Community Mental Health Nurse (Band 6 RMN)
Posted 04 September 2025 by TiaHallam Medical has exciting opportunities for Band 6 Senior Community Mental Health Nurses to join Dementia Assessment and Support Services across Shropshire.
We are seeking experienced and compassionate NMC Registered Community RMNs to deliver high-quality clinical services, supporting patients and their families in the community.
As a Senior Community Mental Health Nurse, you will:
- Undertake comprehensive clinical assessments in line with the dementia pathway, identifying needs, risks, goals, and strengths.
- Develop and implement formulation-based care plans in collaboration with patients, families, and wider teams.
- Deliver tailored nursing interventions to individuals experiencing mental health-related difficulties.
- Work primarily within the dementia community mental health pathway, with flexibility to support across pathways where required.
Skills / Experience:
- Registered Mental Health Nurse (RMN) with active NMC registration.
- Post-registration experience, supported by evidence of further study or training at diploma/degree level.
- Experience of working with individuals who have complex mental health needs and may be hard to engage with services.
- Proven ability to work autonomously within a community setting.
Shifts available: 09:00 – 17:00.
