Advanced Clinical Practitioner (ACP)
Posted 05 November 2025 by RhianeWe are looking for an experienced and motivated Advanced Clinical Practitioner (ACP) to join a dedicated GP Out of Hours (OOH) team in Telford.
This is an exciting opportunity for Advanced Clinical Practitioners to play a vital role in delivering safe, high-quality, and responsive care to patients presenting with urgent and unscheduled healthcare needs outside of core GP hours.
You will work as an autonomous practitioner, assessing, diagnosing, and managing a wide range of acute presentations. You will be supported by an experienced multidisciplinary team, including GPs, nurses, and clinical coordinators.
Key Responsibilities:
- Provide high-quality, evidence-based clinical assessment and management for patients in the OOH setting.
- Make autonomous decisions in relation to patient care, including prescribing, referrals, and discharge planning.
- Work collaboratively within a multidisciplinary team to ensure continuity and quality of care.
- Use advanced clinical skills to triage, diagnose, and treat patients presenting with undifferentiated and complex problems.
- Contribute to clinical governance, audit, and service improvement initiative

Essential Criteria:
- Registered with the NMC or HCPC.
- Hold a Postgraduate Diploma or MSc (or equivalent) in Advanced Clinical Practice.
- Independent Prescriber qualification (V300) is highly desirable.
- Proven experience in an primary care setting.
- Strong clinical decision-making, communication, and interpersonal skills.
- Ability to work autonomously and as part of a multidisciplinary team.
