We are looking for autonomous ACP’s with experience in community, urgent care or acute settings to support this client. This is a brilliant opportunity to support rapid community-based care, help prevent avoidable hospital admissions, and make a real difference to patients in their own homes.

Requirements
- Qualified ACP with experience in community, urgent care or acute settings
- Confident in autonomous assessment, diagnosis and treatment
- Strong clinical decision-making and ability to work independently in the community
- Passionate about delivering patient-centred care
