First Contact Physiotherapist (FCP)

At Hallam Medical, we’re looking for First Contact Physiotherapists to support a Primary Care Network across Lewisham. 

The post-holder will provide a high-quality first-contact physiotherapy service for a Primary Care Network population of approximately 40,000 patients across Lewisham and surrounding practices. Working both face-to-face and remotely, the physiotherapist will assess, diagnose, plan, implement, and evaluate treatment and rehabilitation for patients with musculoskeletal and other relevant conditions. The role includes managing appointment scheduling, using EMIS for clinical documentation, and delivering patient-centred, evidence-based care.

Key Responsibilities:

• Assess, diagnose, and deliver evidence-based physiotherapy treatment plans.
• Manage and schedule new and follow-up appointments.
• Provide consultations both face-to-face and remotely.
• Demonstrate exercises and promote self-management strategies.
• Record all clinical activity accurately on EMIS.
• Work autonomously, escalating concerns when needed.
• Liaise with GPs, practice staff, AHPs, and partners to ensure smooth patient pathways.
• Participate in MDT meetings and contribute to service development.

 

Service & Operational Duties:

• Work flexibly across daytime, evenings, and weekend sessions.
• Maintain accurate appointment booking and documentation.
• Contribute to KPIs and continuous service improvement.
• Ensure safe use and maintenance of physiotherapy equipment.
• Adhere to all governance, safeguarding, infection control, and confidentiality requirements.
