Advanced Clinical Practitioner (ACP)

We are keen to hear from dedicated and qualified Advanced Clinical Practitioners who have experience supporting an Acute Visiting Service.
As an Advanced Clinical Practitioner you will provide necessary assessments and treatments to all ages to help manage their conditions effectively without the need for hospital admission.
The Service Operates
- Out of Hours (OOH) Service, evening and weekends – which provides essential urgent care to patients when they need it most.
- In-Hours Acute Home Visiting Service, Monday – Friday 9am – 18.30pm – delivering acute assessment and care to patients in their own homes.
Client Requirements
- NMC or HCPC Registration
- Full Master Degree
- See all ages – Desirable
- Prescribing
- Experience supporting an Acute Visiting Service
