Hallam Medical Registration

To complete your registration, please provide the following:

• CV – Your most up-to-date CV, including your PIN number, Smartcard number and all qualifications.
• Declarations Form – Please review and complete the form here.
• Qualifications – Evidence of all relevant professional qualifications.
• ID Documents – One form of photo ID (passport or driving licence) and one proof of address.
  • If you do not have a driving licence, please provide two proofs of address instead. Acceptable documents include a council tax bill (dated within the last 12 months) or a utility bill/bank statement (dated within the last 3 months).
  • If you have ever changed your name, please also provide supporting evidence (e.g., marriage certificate or deed poll)
• References – Two professional (clinical) references covering the last 3 years of employment. These must be from line manager level or above and from professional email addresses.
• Medical Indemnity Insurance – Proof of current membership. If you are with the RCN, you can download a confirmation letter from your account under My Downloads. If you are with another provider, please request confirmation directly from them.
• Enhanced DBS Certificate – This must be registered with the DBS Update Service. If you do not have a DBS, we can apply for one on your behalf for £67.10, which will be fully refunded once you have completed 65 hours of work. The Update Service costs £16 per year.
• Occupational Health Questionnaire – Please complete the form here and provide evidence of the immunisations listed.
• ID Photo – A recent passport-style photo. This must be clear, unedited, and without filters.
• Mandatory Training (CSTF Aligned) – Please send any in-date certificates you already have. You can complete any missing modules using our training link here.
