Advanced Clinical/Nurse Practitioner (ACP/ANP)

In this role you will provide high-quality, patient-centred care to people in their own homes across York. This is a rewarding opportunity to work autonomously, support local GP practices, and deliver excellent clinical care to patients who need it most.
Role Overview:
- Conduct home visits for acute and chronic conditions
- Perform assessments, diagnoses, and management plans
- Prescribe safely and appropriately
- Work collaboratively with local GP practices and community services
- Deliver compassionate care to patients who may be frail, housebound, or unable to attend appointments
Required Qualifications & Experience
- NMC/HCPC Registration
- Master’s (MSc) in Advanced Clinical Practice or equivalent Level 7 qualification.
- V300 Independent Non-Medical Prescribing
- Experience in primary or urgent care
- Strong clinical decision-making skills
