Advanced Clinical Practitioner (ACP)
Posted 23 September 2026 by UrsulaHallam Medical has exciting opportunities for experienced Advanced Clinical Practitioners (ACPs) to join an Out of Hours night service across West Bristol.
We are keen to speak with confident, autonomous ACPs to support a busy night service, delivering care through base appointments, telephone triage and home visits.
In this role, you will assess and manage patients presenting with undifferentiated illness, complex needs, and a range of acute and chronic conditions.
You will utilise your advanced clinical assessment and reasoning skills to make diagnoses and prescribe, delivering high-quality patient care in an autonomous setting.
This opportunity is outside IR35.
Essential Requirements
- NMC / HCPC registered
- Advanced clinical assessment
- Reasoning and decision-making capability
- Independent prescribing qualification
- Experience in triage, base appointments and home visiting
- Proven experience working in general practice, minor illness, urgent or emergency care, community care or an out of hours service
- Able to independently assess, diagnose, treat, and discharge
- Strong autonomous decision-making and triage skills
