Advanced Clinical Practitioner (ACP)
Posted 21 October 2025 by LiamHallam Medical has opportunities for Advanced Clinical Practitioners (ACP) to support an Out of Hours service based in Liverpool, Merseyside.
We’re looking for Advanced Clinical Practitioners with urgent care or primary care experience to support our client in Liverpool.
Key Responsibilities:
- Conduct clinical telephone triage, ensuring patients receive timely and appropriate care.
- Work closely with the medical and nursing leadership team to coordinate patient care.
- Review patients awaiting contact, actively identifying risks and signposting to appropriate services.
- Use a Red Flag triage approach to prioritise cases in line with service escalation plans (including elderly and palliative patients).
- Provide safe and effective transfer between clinical dispositions, applying strong clinical reasoning skills.
To apply for this role:
- Urgent Care / Primary Care Experience
- Msc in Advanced Clinical Practice, PGDip or Equivalent
- Clinical Examination and Diagnostics Level 6 or 7
- V300 Prescriber
- NMC Registered
