Advanced Community Practitioner(Band 7)
Posted 12 June 2026 by UrsulaHallam Medical has opportunities for Advanced Community Practitioners (Band 7) to join an Urgent Response Team in Medway.
As an ACP, you will work as part of the multidisciplinary Urgent Response Service to support unwell patients in domiciliary settings and care homes.
You will undertake face-to-face assessment, diagnosis and treatment of community patients who present as unwell, helping to prevent hospital admissions where appropriate and ensuring patients remain safe within their own homes.
In addition, Advanced Community Practitioners case-manage individuals whose conditions are impacting their quality of life. These individuals may be living with frailty and/or one or more long-term conditions. Following assessment, you will facilitate access to appropriate services, both within and external to the organisation, and evaluate outcomes in collaboration with the patient’s GP and other healthcare professionals.
To Apply:
- NMC or HCPC Registered
- Must be a prescriber
- Must have community experience
- Essential to have RIO experience
- Enhanced Clinical Assessment Skills
- Experience in the fields of elderly/ rehabilitation/ community/ intermediate care/ long term conditions
- Proven and evidence of experience in an autonomous practitioner role
