We are seeking an experienced and highly motivated Advanced Practitioner to support our community team in Brighton. This is an exciting opportunity to work within a proactive multidisciplinary service delivering high-quality care to care home residents and housebound, frail patients in the community.
The post holder will provide both acute and chronic care management, including planned ward rounds and same-day urgent visits, helping to reduce unnecessary hospital admissions and improve patient outcomes.
Key Responsibilities:
- Provide advanced clinical assessment, diagnosis and treatment for care home residents and housebound frail patients
- Undertake ward rounds and same-day acute visits as part of the community service
- Manage both acute presentations and long-term conditions
- Work autonomously within an agreed scope of practice
- Maintain accurate and timely clinical records
- Work collaboratively within a multidisciplinary team to deliver holistic, patient-centred care
You Will Need:
- Registered healthcare professional (NMC / HCPC)
- Advanced Practitioner qualification or working towards MSc ACP
- Experience in community, urgent care or primary care settings
- Confident using EMIS and SystmOne (S1)
- Full driving licence and access to own vehicle
- Ability to work independently and manage complex clinical needs
- Excellent communication and organisational skills
