Advanced Practitioner (ANP/ACP)
Posted 14 July 2026 by SheridanHallam Medical has an opportunity for Advanced Practitioners (ANP/ACP) to join an Integrated Urgent Care and Reablement Service in Nottinghamshire.
We are looking for dual-skilled ANPs and/or ACPs to work within the Integrated Reablement Team (IRT) / Virtual Ward (VW) to assess, diagnose, treat or refer patients aged 18+, presenting with undifferentiated and undiagnosed problems.
You will provide clinical assessment and deliver treatments to service users who have undifferentiated health care requirements in order to support the services delivered by the IRT/VW and prevent unnecessary acute hospital attendances and support early discharge.
This position offers a variety of shift patterns, available 7 days a week.
Client Requirements:
- Hold a NMC or HCPC Registration
- Competent in managing Minor Illness Level 7
- Home Visiting experience
- Recognised full Master’s level qualification in Advanced Clinical Practice
- Evidence of ability to apply theory to practice
- Recognised teaching/mentorship/Supervision Qualification
- Evidence of autonomous clinical practice in a community setting
- Non-medical Prescriber (V300)
- Advanced physical examination skills
- Competence working within the Integrated Reablement Team (IRT)/Virtual Ward (VW)
