We are seeking an experienced and motivated Advanced Practitioner to join our friendly and supportive GP Out of Hours and Home Visiting service in Hereford. This is a predominantly face-to-face clinical role, providing high-quality urgent primary care to patients during across the week, primarily evening nights and weekends.
You will work as part of a multidisciplinary team delivering safe, effective and patient-centred care in a modern clinic environment.
Key Responsibilities:
- Provide autonomous clinical assessment, diagnosis and treatment of patients presenting with urgent primary care needs
- Manage acute and long-term conditions within your scope of practice
- Prescribe safely and appropriately
- Refer patients to other services as required
- Work collaboratively with GPs, nurses, paramedics and operational teams
- Maintain accurate and timely clinical records
- Ensure patient safety and high standards of clinical governance
You will need:
- A qualified Advanced Clinical Practitioner/Advanced Nurse Practitioner/Advanced Paramedic Practitioner
- V300 Qualification
- Registered with your relevant professional body (NMC/HCPC)
- Experienced in Primary Care settings

- Palliative Care Experience
- Confident working autonomously in a face-to-face clinic environment
- An excellent communicator with strong clinical decision-making skills
