Hallam Medical have an exciting opportunity for Advanced Practitioners to support a GP OOH and Home Visiting service in Hereford

We are seeking an experienced and motivated Advanced Practitioner to join our friendly and supportive GP Out of Hours and Home Visiting service in Hereford. This is a predominantly face-to-face clinical role, providing high-quality urgent primary care to patients during across the week, primarily evening nights and weekends.

You will work as part of a multidisciplinary team delivering safe, effective and patient-centred care in a modern clinic environment.

Key Responsibilities:

  • Provide autonomous clinical assessment, diagnosis and treatment of patients presenting with urgent primary care needs
  • Manage acute and long-term conditions within your scope of practice
  • Prescribe safely and appropriately
  • Refer patients to other services as required
  • Work collaboratively with GPs, nurses, paramedics and operational teams
  • Maintain accurate and timely clinical records
  • Ensure patient safety and high standards of clinical governance
