Posted 28 October 2025 by Reece JamesHallam Medical has an exciting opportunity for an experienced Advanced Clinical Practitioner (ACP) to support a Urgent Community Response Team (UCR) In Chichester.
As a experienced Advanced Clinical Practitioner, you will play and integral part in the Urgent Community Response Team, were you will provide expert clinical assessment, diagnosis, and management for adults experiencing acute health or functional crises in the community.
The role aims to prevent unnecessary hospital admissions by delivering timely, person-centred care within patients’ homes or usual place of residence, supporting the principles of “home first” and integrated care.
To apply for this role:
- Msc in Advanced Clinical Practice
- Must hold a full Digital Badge
- Must be a car driver with business insurance
- Must have experience in admission avoidance in the community
- V300 Prescriber
- NMC Registered or HCPC Registered
