Hallam Medical have a brand new opportunity for a Band 7 Community Natron to join a community-based service in Bedford.

We are seeking an experienced and motivated Band 7 Community Matron to join a community-based team in Bedford. In this pivotal role, you will provide proactive case management for patients with long-term conditions and minor illnesses within care homes and home environments, promoting independence and high-quality care close to home.

Key Responsibilities:

  • Provide proactive case management for patients with long-term conditions and/or minor illness in care homes and home settings.
  • Support the 2-hour response/Virtual Ward requirements, completing timely physical assessments in the community.
  • Educate and enable patients and carers to manage their conditions, ensuring discharge within 30 days following agreed patient pathways.
  • Coordinate care as part of the local Working Together collaborative.
  • Proactively assess, monitor, and coordinate care for patients with long-term conditions or minor illness.
  • Ensure timely, effective multi-agency input to meet patient needs within the community.
  • Contribute to primary care–led monthly MDTs, supporting decision-making on care and service provision.
  • Reduce reliance on secondary care by increasing care delivery in primary, community, and home settings.
  • Respond to urgent review requests from the Primary Care Home team.
  • Make daily contact with aligned care homes to identify patients requiring review or assessment.

 

Clinical Requirements:

  • Safely undertake moving and positioning of patients and equipment in line with manual handling policies.
  • Carry out clinical procedures within scope of competence using appropriate clinical instruments and equipment.
  • Use highly developed clinical skills, including independent prescribing (following accredited training).
  • Deliver evidence-based care in a range of settings, some of which may be unpredictable.
  • Prioritise workload effectively to ensure continuity of care and meet organisational deadlines.
  • Maintain high standards of infection control, ensuring safe disposal of contaminated materials.
  • Provide safe clinical care in patients’ homes, GP surgeries, health centres and other community venues, including in variable or potentially hazardous environments.
