Role Overview:
- Deliver patient centered clinical care and support patients in their own home, residential settings or clinics
- Deliver treatments like wound care, end of life care and phlebotomy
- Support patients that have complex or long term conditons
- Make healthcare plans to suit a patients needs
- Be able to work closely with the GP’s, social workers and other healthcare practitioners
Skills Required:
- Blood Pressure
- Catheterization
- Diabetic Monitoring
- Home Visits
- IV Medicines
- Home Visits experience
- Palliative Care- End of Life/ Palliative care Visits
- Respiratory experience
- Simple Dressings
- Wound Care
Client Requirements:
- Must have experience of lone working
- 6 months Community Nursing experience
- UK Driving Licence and access to own vehicle
