Continuing Healthcare (CHC) Nurse Assessor

As a Continuing Healthcare Nurse Assessor, you will play a key role in ensuring patients receive the right care and support at the right time.
You will be assessing, developing and overseeing a personalised care package for those with complex or long-term conditions both in the community and care homes.
Shifts are available Monday to Friday 09:00 – 17:00 with ASAP starts available.
Essential Criteria
- Registered nurse with valid NMC registration
- Previous community experience
- 6+ months experience
- UK Driving Licence
- Experience working in continuing healthcare
- Experience supporting patients with complex needs
- Strong assessment, communication and organisational skills
